Our Active Ageing Centres (AAC) serve as a key point of contact for seniors under our care. We proactively reach out to seniors, organise social activities and provide opportunities for seniors to interact with members of the community, with the aim to enable seniors to adopt an active lifestyle, pre-empt social isolation, and refer them to assistance in a timely fashion.

Objectives

To improve the health outcomes of seniors living in the community through provision of a suite of programmes to help seniors live healthier lives and support their aspirations to age in place.

Our Services

  1. Outreach and Community Collaboration
  • We conduct general outreach (e.g. posters, collaterals and events) to better understand the needs of seniors so as to provide suitable services and increase community awareness of our AACs’ services.
  1. Active Ageing
  • We provide a drop-in social recreational space for clients.
  • Conduct Active Ageing Programmes (AAPs), or refer clients to AAPs organised by other organisations in the community.
  • Render assistance with ad-hoc tasks (e.g. reading letters, changing lightbulb) or Instrumental Activities of Daily Living (IADL) where possible.
  • Monitor and respond to Alert Alarm System (AAS) calls for HDB flats with AAS installed within our service boundary.
  1. Buddying and Befriending
  • We provide buddying/befriending services to seniors who are
  • Living alone
  • Have limited or no social support
  • Caring for someone with high care needs
  • Have limited social interaction due to physical or cognitive impairment
  • During each buddying/befriending session, befrienders may engage seniors in mid-stimulating activities such as learning how to use the smart phone or playing games, promoting healthy living, or simply lending seniors a listening ear.
  1. Information and Referral
  • We provide information on relevant schemes, grants and support available for seniors and their caregivers and refer seniors to appropriate organisations if seniors require additional community care and social support services beyond the AAC’s capability (e.g. day care, case management)
