Active Ageing Centres
Active Ageing Centre Department serves as a key point of contact for seniors under their care. These AACs proactively reach out to seniors, organise social activities and provide opportunities for seniors to interact with members of the community, with the aim to enable seniors to adopt an active lifestyle, pre-empt social isolation, and refer seniors to assistance in a timely fashion. AACs’ baseline services include:

(iii) Buddying and Befriending
(iv) Information and Referral
Active Ageing Centres
Community Case Management Service
Community Case Management Service (CS) aims to enable vulnerable and low-income seniors who have no or low family support living within the service boundaries to age in place within the community for as long as possible
Advance Care Planning
Advance Care Planning (ACP) is the process in planning for your future health and personal care. Having ACP conversations allows you to:
- Share your personal values and beliefs
- Explore how your values and beliefs affect your healthcare preferences in difficult medical situations
- Think about who among your loved ones can be your voice if you become very ill one day
Community Palliative Care, Cloud 9
Cloud 9 Community Palliative Care aims to help seniors who are diagnosed with a serious or life-limiting illness. The programme targets to help these seniors navigate complex decisions and receive community support. Therapeutic activities, group outings and counselling will be conducted within Fei Yue Active Ageing Centres (Buangkok Green and Limbang Green).
Integrated Community Care Provider (ICCP) @ Fei Yue Community Services
ICCP brings together Active Ageing Centres (AAC), Senior Care Centres (SCC), Home Personal Care (HPC+), Home Therapy (HT) providers and other community agencies to strengthen partnerships and improve care coordination. Through this collaborative approach, seniors can be linked to appropriate services in a timely manner and receive holistic support that enables them to remain active, connected and independent while ageing in place.

Elder Education Department
Elder Education Department provides a gamut of active ageing programmes that reaches out to seniors in the community. To encourage seniors to maintain active lifestyles, the department offers a wide variety of programmes to promote lifelong learning, healthy living, social networking and inter-generational bonding. Their efforts to encourage seniors to adopt digital technology is a key thrust to give seniors more options – in the way they receive information and how they communicate with others. The department believes that learning never stops.
