Cloud 9 Community Palliative Care is initiated and supported by the Lien Foundation in partnership with Fei Yue Community Services. It serves seniors aged 60 and above who are living within Buangkok Green and Limbang Green regions and are diagnosed with a serious or life-limiting illness, such as cancer, organ failure, Parkinson’s, and stroke.

Services Provided:

Social support through therapeutic activities, talks and group outings.
Individual and group work which include grief and bereavement counselling for seniors and caregivers with psychological distress
Home palliative care: Medical, nursing and occupational therapy services for early stages of life-limiting or terminal illnesses
Guidance on making complex decisions such as Advance Care Planning, Lasting Power of Attorney and Will
Referral to community resources such as case management and home care, as well as disease-specific support groups
