Home Medical
Home-bound seniors with chronic conditions or disabilities who require continuing or long-term medical care.
This service includes:
- Chronic Disease Management
- Medication Management (i.e. prescription, medication administration and medication reconciliation)
Home Nursing
Homebound clients with chronic conditions or disabilities who require specific nursing care/procedures that can only be provided for by a trained nurse.
This service includes:
- Management and care of tube (NG tube and catheter)
- Wound care
- Stoma care
- Tracheostomy care
- Medication Packing
- Medication Administration
- Monitoring of chronic diseases
- Health Education
- Caregiver training and support
Enhanced Home Personal Care (HPC+)
Homebound clients who are assessed to require assistance in their Activities of Daily Living (ADL) and/or Instrumental Activities of Daily Living (IADL), subject to the Approved Provider’s assessment of the client using the InterRAI assessment tool.
