Job title: Resource Case Manager
Roles & Responsibilities
About Community Case Management Services:
Our CCMS service aim to deliver holistic care that is centred on senior’s needs. We work with seniors who are frail, whom have multiple care needs, that will become increasing complex with time. With the integration of appropriate use of community resources, it enables the seniors to continue to stay in the community for as long as possible, therefore allowing them to age-in-place.
Responsibilities:
- Provide counselling & case management services
- Develop care plan for clients and/or caregivers
- Conduct monthly session (phone follow up and face to face) with clients and/or caregivers/family members
- Conduct assessments and provide documentation of the cases.
Requirements:
- Degree in Social Work/Counselling or Diploma in Social Work/Counselling with minimum 1.5 years of work experience
- Must have passion to work with seniors and caregivers.
- Relevant experience in working with seniors preferred
- Good communication and interpersonal skills
- Proficient in Microsoft Applications (Word, Excel, PowerPoint)
- Job position is part-time and contractual basis
Job title: Medical Escort (Contract)
Roles & Responsibilities
We are seeking a compassionate and committed individual to support our Community Case Management Service Programme. This position involves hands-on support as a medical escort, helping to ensure a positive and safe experience for our clients.
Responsibilities:
- Accompany clients to and from medical appointments (e.g. hospitals, clinics, therapy sessions).
- Ensure clients’ safety during transit, including providing wheelchair assistance where needed.
- Offer emotional support and reassurance to clients during appointments.
- Assist with registration, navigation within healthcare facilities, and simple administrative tasks.
- Communicate key updates or observations to case managers and caregivers.
- Maintain accurate records of escort services provided.
- Adhere to all safety, confidentiality, and infection control protocols.
Job payment of $30 is based on a medical escort trip is completed.
Requirements:
- GCE O Level / A Level / Diploma
- Good communication and interpersonal skills
- Bilingual or multilingual skills (English, Mandarin, Tamil and Malay) or dialects
- Patient, empathetic, and service-oriented
- Comfortable interacting with seniors and/or vulnerable individuals
- Able to handle sensitive information with discretion
- Basic administrative and documentation skills
- Physically fit and competent to assist clients with mobility needs
- Prior experience in healthcare, community services, or customer service will be an advantage (but not essential)
- Working independently
Job title: Client Satisfaction Interviewer (Contract)
Roles & Responsibilities
We are seeking a compassionate and committed individual to support our Community Case Management Service Programme. This position involves client engagement through satisfaction surveys.
Responsibilities:
- Conduct phone-based interviews using structured questionnaires to collect client feedback.
- Engage clients in a professional, respectful, and empathetic manner, ensuring confidentiality.
- Accurately capture and record responses while maintaining high data quality.
- Require stating reason(s) for ratings that fall below the minimum stipulated scores
- Submit completed questionnaires in a timely manner.
- Working hours is ad-hoc. Depending on client’s/caregiver’s needs, the call has to be made during or outside of office hours.
Job payment of $8.00 will be paid after a Client/Caregiver Satisfaction Survey is completed.
If a successful call is made, in which client/caregiver picked up and conversed, but there are valid reasons to explain why the client satisfaction questionnaire still cannot be done, then job payment can still be made.
