Community Mental Health Department

COMIT Fei Yue

You can approach over 100 community mental health teams in the community islandwide such as the Community Intervention Team (COMIT). We provide psychotherapy and psychosocial interventions such as cognitive behavioural therapy and psychoeducation support for clients with mental health issues and dementia, and their caregivers. We also work closely with the General Practitioners, polyclinics and hospitals and other community partners to provide collaborative and coordinated care to clients and caregivers in the community.

How can COMIT support you?
  • Provide assessment, psychotherapy, psychosocial intervention such as cognitive behavioural therapy and psychoeducation, to support the reduction of symptoms for clients to enable them to live well in the community, and better support the caregiving journey of the caregivers.
  • Provide collaborative and coordinated care for clients and caregivers through a case management approach.
  • Work in partnership with health and community care partners, and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.
Target Clientele

COMIT serves clients aged 18 and above who are suspected of or diagnosed with mental health condition and or dementia as well as their caregivers and are residing within these constituencies:

  • Bukit Batok Sub-region 3
  • Choa Chu Kang + Lim Chu Kang + Western Catchment + Tengah
  • Jurong West – Pioneer – Sub-region 5

CREST Fei Yue

There are over 100 community mental health teams in the community islandwide, such as the Community Outreach Team (CREST), who serve as your in-person First Stop for Mental Health. They will listen to your concerns and provide support such as therapy to support your mental well-being.

How can CREST support you?
  • Promote early recognition of signs and symptoms of mental health conditions and/or dementia.
  • Improve mental health conditions and/or dementia literacy by organising targeted outreach events and activities to provide mental health and/or dementia information and education to residents and caregivers in the region.
  • Provide psychosocial intervention such as techniques to reduce the risk of escalation of mental health needs.
  • Provide collaborative and coordinated care for clients and caregivers through a case management approach.
  • Work in partnership with health and community care partners and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.
Target Clientele

CREST serves clients age 18 and above who are at risk of/diagnosed with mental health conditions (e.g., depression and dementia) as well as their caregivers and are residing within these service boundaries:

CREST-PDS Fei Yue

The Post-Diagnostic Support Community Outreach Team (CREST-PDS) provides education on the condition, coordinates community resources and facilitates early planning and management of the condition for newly diagnosed persons living with dementia and their caregivers for six months from the time of referral.

How can the CREST-PDS Team support you?
  • Provide psychoeducation and caregiving advice to persons newly diagnosed with dementia and their caregivers. Promote identification of caregiver and client needs and link up to appropriate support services.
  • Provide psychosocial intervention with care planning and regular follow-up with clients and caregivers.
