About Community Mental Health Department:

Our Community Resource, Engagement and Support (CREST) teams support clients who are at risk of or diagnosed with mental illness as well as their caregivers. The teams provide basic emotional support and monitoring of their mental health conditions through home visits and service linkages.

Responsibilities: 

  • Assess the needs of clients and/or their caregivers, customize care plans, and provide relevant service linkages within the healthcare and social service sector 
  • Monitor the conditions of clients and/or caregivers of persons with dementia and provide them with relevant emotional and cognitive support 
  • Raise awareness on mental health by engaging caregivers in psychoeducation efforts (e.g. trainings, workshops, broadcasting) 
  • Support the run of the PDS Caregiver Support Network 
  • Ensure proper maintenance of case documentation 

Requirements: 

  • Diploma Holder 
  • Passionate to work in the mental health sector and community services 
  • Strong interpersonal and communication skills 
  • Must be willing to work on weeknight and/or Sat morning (when necessary) 
  • This position is on contract basis
